Well, the game IS very difficult. Mainly because you're trying to get through a dungeon full of spikes, pitfalls, guillotines, and dangerous guards with lots of health. Your character can only fall 2 floors safely - a fall of 3 floors or more is fatal. There are a bunch of confusing gate puzzles. In addition, you only have one hour to complete all of the dungeon or your girlfriend dies and you get the "bad ending". And your time doesn't reset if you die - and you will die a lot - which sucks. Also you only have 2 buttons to use, due to NES restrictions.
